Senior Healthcare Centers are outpatient departments of North Florida Regional Healthcare dedicated to providing comprehensive primary physician care to seniors on Medicare. Our team consists of physicians, nurses, case managers and other healthcare professionals all dedicated to meeting the individual needs of patients 65 and older.
The nurse role consists of completing intake vitals, medication reconciliation, assessments, reviewing discharge instructions, and coordination of ordered treatments and referrals for patients being seen in the clinic. Other tasks may include triage phone calls, patient education, and coordination of services with community resources, completing venipunctures and waived testing. All documentation is completed within an electronic medical record.
EDUCATION REQUIRED:
Graduation from an accredited school of Nursing.
EXPERIENCE REQUIRED:
1 year related experience. Geriatric or clinic care experience preferred.
SKILLS, KNOWLEDGE, AND ABILITIES:
Demonstrated skills in organization, verbal and written communications, customer service, math, analysis, reading, following instruction, proficient computer usage. Skilled in handling fast pace and multiple priorities.
LICENSURE/CERTIFICATION:
RN license in the State of Florida.
BLS.
